Unlocking Opportunities: The Importance of the UDID Card
Every student deserves a smooth journey through their education and daily life. For our students with special needs, the Unique Disability ID (UDID) is a vital tool that makes this journey easier. Often called the “Swavlamban Card,” it is much more than just an ID—it is a key to empowerment and inclusion.
1. What is the UDID?
The UDID is a single, national identity document for persons with disabilities issued by the Government of India. Traditionally, individuals had to carry multiple paper certificates for different departments. The UDID replaces these with one smart card that works across the entire country, ensuring that data is digitised and easy to verify.
2. Why is it Beneficial?
Having a UDID card opens many doors for students and their families:
- Easy Access to Schemes: It is the primary document required to avail of government scholarships, grants, and aids.
- Travel Concessions: It allows for discounted fares in Railways and State Road Transport buses.
- Track Progress: The card helps the government track the delivery of benefits, ensuring they reach the right person at the right time.
- Universal Validity: Whether you are in your home state or travelling across India, the card is recognised everywhere, eliminating the need to carry bulky original documents.
3. How to Apply: A Step-by-Step Guide
The process is designed to be transparent and is completed in three main phases:
Phase A: Online Registration
- Visit the official portal: www.swavlambancard.gov.in.
- Click on “Apply for Disability Certificate & UDID Card.”
- Fill in personal details, address, and disability information.
- Upload required documents (Photo, Signature/Thumb impression, and Address Proof).
- Submit the form and note down your Enrolment Number.
Phase B: Medical Assessment
- Once the application is submitted, it is forwarded to the local Government Hospital.
- You will receive a notification (via SMS or email) with a date for a medical assessment.
- The individual must visit the hospital for a physical check-up by a specialist doctor to verify
the percentage of disability.
*If you do not receive a notification, you should visit the hospital you selected during the
application process to proceed with the assessment. As the visit is required to complete the medical
assessment for your disability ID.*
Phase C: Receiving the Card
- After the medical check-up, the hospital uploads the results on the UDID portal.
- The digital certificate is generated online.
- The physical UDID card is printed and sent directly to your registered address via speed post.
4. Renewals
UDID cards are generally issued with a specific validity period (often 5 to 10 years) or are marked as “Permanent.” If your card has an expiry date, you should apply for a renewal via the same portal at least three months before it expires. The process involves a quick re-verification by a medical authority to update the records.

If you need help scanning documents or filling out the form, please visit the school office during working hours.
To ensure a smooth application process when visiting the school office for assistance, please keep the following documents ready:
- Passport-Size Photograph: A recent, high-quality colour photograph of the student.
- Proof of Identity: A scanned copy of the student’s Aadhaar Card is highly preferred. Other acceptable documents include a PAN Card, School ID, or Passport.
- Proof of Address: This can be the Aadhaar Card, Voter ID, a recent Utility Bill (electricity or water), or a Ration Card.
- Medical Records: If you already have a Disability Certificate issued by a government hospital, please bring it. If not, any existing medical reports or diagnostic records from a government hospital are essential for the medical board’s review.
- Relationship Proof (for children): A Birth Certificate or a School Identity Card (signed by the Principal and containing parents’ names) to verify the relationship with the parent or guardian.
- Category/Caste Certificate (if applicable): A scanned copy of the SC/ST/OBC certificate if you are applying for specific reserved category benefits.
- Income Certificate (if applicable): This is only necessary if you are applying for income-based benefits like specific scholarships.
- Signature or Thumb Impression: A clear scanned image of the student’s signature or thumb impression is optional but helpful for the online form.
